FLORENCE, SC (WMBF) - Florence County School District Three will hold a pre-registration and screening for kindergarten for four and five-year-olds on Thursday, April 24 and Friday, April 25 from 8 a.m. until 4 p.m.
To be eligible the child must be four or five-years-old before September 1, 2013. The parent or guardian must bring the child to registration for screening. Registration is by appointment only. Call the school for which the child is zoned to make an appointment.
The following documentation is required for registration: legal birth certificate, child's Social Security card, Medicaid card (if applicable) or proof of income if the child does not receive Medicaid, state certificate of immunization or exemption card, three-to-four emergency contact names and telephone numbers (other than parents or guardians), proof of residency (utility bill), photo identification of parent or guardian (driver's license or other state issued identification) and legal documentation for anyone other than biological parents to show guardianship.
Certificate of immunization can be obtained from the local Health Department Monday through Friday from 8:30 a.m. until 5 p.m.
